Health Form (must be completed by a doctor based on a physical examination within last calendar year, signed and mailed to camp with a photocopy of both sides of your health insurance card.
Health Center Medication Cards (if your child will be taking any medications while at camp, print this form, fill it out and bring it with you to camp. It will speed you through the health center on opening day!)
